Same Day (Urgent) Referral

Our clinic has introduced a new referral program where some specialty clinics are accepting same-day, Non-Emergency Urgent Referrals.

Provided that it is medically necessary.

a) Submit via our website by clicking here

b) Fax referral to: (905) 897-7780.

c) Patient can bring a hard copy of referral during their appointment.

a) If the patient has a signed referral in hand (hard copy), they can simply walk-in to our clinic between the hours 9AM to 9PM, 7 Days a week and see a general internist or general pediatrician. In the case of general surgery or gynecology the patient needs to call the clinic at 905 897 9228 to ensure that the gynecology and general surgery clinic can accommodate them coming in on a walk-in basis. .

b) Once the Referring Doctor has faxed us a referral, patient is welcome to call our appointment booking line (905) 897 - 9228 to book an appointment.

a) Patient health card (if applicable)

b) Patient referral (only if it has NOT been sent to our office previously)

c) Any relevant test reports (only if it has NOT been sent to our office previously)
