These specialty clinics run by appointment. Our waiting times are relatively shorter than most standalone specialist offices. Most specialty clinics can be accessed within a week.
How to see a medical specialist at our facility?
a) Patient or Physician can print above form and have a physician fill & sign referral.
a) Once we have received a referral, patient can call our appointment booking line (905) 897 - 9228 to book an appointment.
b) If the patient has a signed referral in hand (hard copy), they can also call our appointment booking line (905) 897 - 9228 to book an appointment. On the day of the appointment, patient will have to provide the front desk staff the signed referral during registration.
